Your Rights Regarding PHI

  1. Right to Request Limits on Use and Disclosure: You can request restrictions on the use or disclosure of your PHI for treatment, payment, or health care operations. While we are not required to agree to all requests, we will consider them carefully and notify you if we cannot comply.
  2. Right to Restrict Disclosures for Fully Paid Services: You have the right to request that we not disclose PHI to your health plan for items or services you have paid for in full out-of-pocket.
  3. Right to Choose How We Contact You: You may request specific ways we contact you (e.g., home or office phone, specific mailing address), and we will accommodate reasonable requests.
  4. Right to Access Your PHI: You have the right to request an electronic or paper copy of your medical record and other PHI. We will provide a copy or summary (if agreed upon) within 30 days of your request and may charge a reasonable, cost-based fee.
